Chapter Membership Form and Permission Forms

IBF Chapter Volunteer Membership FormUse the following forms to join a chapter and volunteer with IBF (at school and offsite). The chapter President and Secretary should verify and ensure that every member form is completed fully and signed by member and parent, before allowing the member to join any activities.
